1. Agreement and Authorization
This Client Service Agreement and Case Assignment Terms (“Agreement”) is entered into by and between Sightline Investigative Solutions (“Sightline,” “Company,” “we,” “us,” or “our”) and the client, insurance carrier, third-party administrator, law firm, business entity, or authorized representative submitting or approving a case assignment (“Client,” “you,” or “your”).
By submitting a case assignment, approving services, checking the acceptance box, sending written authorization, or directing Sightline to begin work, Client agrees to these terms.
3. Scope of Services
Sightline provides professional surveillance, field investigation, and claims-support services by assignment. Services may include surveillance, activity checks, Alive & Well checks, AOE/COE investigations, recorded statements, scene investigations, EDR services, medical canvassing, social sweeps, locates/skip tracing, process serving, SIU/claims support, field inspections, witness development, neighborhood canvassing, report preparation, media handling, and related investigative support.
The specific scope of each assignment is based on the information, instructions, authorization, and materials submitted by Client and accepted by Sightline.
4. Rates, Billing, Mileage, Minimums, and Special Pricing
Unless otherwise agreed in writing, services are billed at $100.00 per hour, portal to portal, plus mileage at $0.72 per mile. Applicable minimums, rush fees, after-hours rates, court testimony rates, and approved or reasonably necessary assignment-related expenses may apply.
Medical canvassing, rush assignments, after-hours work, court appearances, travel, lodging, parking, tolls, records fees, database fees, and other case-specific costs may be billed when applicable or authorized.
5. Client Authorization and Uploaded Materials
Client certifies that it is authorized to submit the assignment, provide any uploaded materials, request investigative services, and receive case information and deliverables related to the assignment.
Client is responsible for providing accurate assignment details, known restrictions, deadlines, representation information, safety concerns, and any limitations affecting the investigation.
6. No Guaranteed Results
Sightline will make reasonable professional efforts based on the assignment information provided. Results, claimant activity, witness cooperation, surveillance opportunities, records availability, or investigative findings are not guaranteed.
7. Confidentiality and Legal Compliance
Services are conducted subject to applicable law, operational feasibility, client authorization, and case-specific requirements. Client agrees not to request or direct illegal, improper, or unauthorized activity.
Case materials, reports, media, and findings are intended for authorized claims, legal, business, or investigative use by the Client and its authorized representatives.
8. Payment Terms
Invoices are due upon receipt unless otherwise agreed in writing. Client agrees to pay approved charges, applicable minimums, mileage, expenses, and authorized services associated with the assignment.
9. Acceptance
By checking the acceptance box and submitting the assignment, Client confirms it has read and agrees to this Agreement, certifies it is authorized to submit the assignment and uploaded materials, and authorizes Sightline Investigative Solutions to proceed with assignment review and approved services.
